The history of eye bag surgery can be traced back a hundred years. From the simple removal of fat in the past to today's precise reshaping techniques, every step has recorded the progress and transformation of plastic surgery.
Evolution of Eye Bag Removal Surgery
People realized a hundred years ago that eye bags made people look older and less attractive, so the initial purpose of the surgery was simply removal. The earliest methods were internal incisions, directly removing excess fat from the inside of the eye; or external incisions, removing excess skin and fat from the lower eyelid. However, this excessive removal often resulted in hollow-eyed, making it look more like malnutrition or lack of sleep.
The results of such surgeries made us realize that the fat in the eye bags should not be completely removed, but should instead be regarded as a resource that can be used to fill other concave areas of the face. This is why we later began to try to move the eye bag fat to the sunken areas, such as the tear trough, and combine it with mid-face lifting to enhance the overall effect. The evolution of these techniques has allowed surgery to not only solve the problem of eye bags, but also improve the overall aesthetics of the facial contour.

"Internal and External" + "Precise Treatment" Surgical Strategy
When dealing with eye bag surgery, Dr. Chen first uses an internal incision to appropriately remove the protruding eye bag fat from the conjunctiva, reducing the damage to the muscles from external incisions, and then uses an external incision to remove the slack skin, restoring the tightness of the lower eyelid. This treatment method has several advantages. Internal incisions can avoid damaging the orbicularis oculi muscle. Excessive incision or removal of the orbicularis oculi muscle may increase the risk of ectropion or exposure of the sclera.
In addition, preserving the integrity of the orbicularis oculi muscle also helps to maintain the shape of the fat pad. The fat pad is located below the eyelashes and is a muscle structure that protrudes when smiling. If the muscle is incised during surgery, it may affect the appearance of the fat pad. The surgery only removes excess skin externally without affecting the muscle, and treats the eye bag fat through internal manipulation. This design not only reduces risks but also preserves natural beauty.

CO2 Laser: Technological Assistance for Surgical Recovery
In eye bag removal surgery, how to shorten the patient's recovery period has been a long-term focus. To this end, CO2 laser technology has been an ideal solution. The wavelength of this laser can be highly absorbed by water molecules, producing local high heat within the tissue, achieving simultaneous cutting and hemostasis. Compared with traditional electrosurgical knives, the heat-affected area of the laser is smaller, reducing damage to surrounding tissues.
Using CO2 lasers can not only reduce intraoperative bleeding but also shorten surgery time. The results confirmed that this technology can significantly accelerate patient recovery, allowing them to return to their normal social life and work more quickly. Dr Chen believes this is one of the future directions of eye bag surgery.
